He also hits the fat guy in the face.

Keep watching it. The fat guy's face is turning briefly toward his own left--then suddenly to the right--along with the direction of beefcake's fist (cuz his face got sideswiped by the fist), and then his fat face resumes its natural course to the left.

Or maybe the fat guy represents unintended consequences from the decisions of Mr. Macho, a.k.a. Piers Morgan.
