Is it a bug that only some people have been given them, yet not others?

You have to win in the game style before your issued what your due.

IE I had to win another fog game before I got my fog medals.

I got my fog medal by winning a game, didn't have to be in that area. A win triggers the script I believe.

Yep that's right, you just need to win 1 game of any type and you will get any game achievement medals that you are due

A couple minutes after I won a fog game a PM popped up saying I won a Fog Medal. It was like Christmas in October! Thanks!

I won a game and got 7 medals. the game type means nothing.
