Close enough. It's actually Ray Jackson, not Jim Jackson, but since he's the one I can never remember, I can let that slide. (the fifth is Jalen Rose)

I think it's (H)akeem Olajuwon and Sam Bowie... pretty sure about the second, not so much the first.

(and NP... there's no need to be super anal about this... all that'll do is prompt people to go look it up on wikipedia, which is really against the spirit of things)
