If the missile is joint-held by myself and a team mate, and we have launch code and president, do we get the +50 bonus? Or do all the pieces need to be held by one player only?

All bonuses on every map apply to 1 player holding it only. Regardless the team ownership, if any.

the instructions state that if any teamates hold a bonus, they won't get it.
