Look at it this way: after each player's turn, the "deck" is reshuffled. Which means that any card you turn in is valid to be given out again, but any card that a player actually owns is not valid to be given out.
In 4545 games, I have never gotten two of the same card/spoil at the same time, nor have I ever heard of anyone posting that they did in either the game chat or in the forums. That's not to say it can't happen, but that's enough for to convince me that it doesn't.
I quite often use the strategy of stacking troops on my spoils in nuclear games because they are safe there, and then moving them when I am going to pick up a fifth card or play my spoils next turn. It's the only safe(ish) way to actually stack troops in a nuclear game.

Each spoil is only in play once except when there are no spoils left. Then a second set is used.
