Tylenol #4 contains paracetamol (abbreviated "APAP"; that's acetaminophen to you americanos) which is extremely harmful to your liver especially when taken in dosages higher than recommended. When you abuse codeine+APAP preparations, you often have to take 5-10x dosages of APAP which if you are sensitive to it (with repeated usage, even if you're not) can cause irreversible liver damage or acute liver failure.

There exist simple methods of separating said substances, but I'm not going to tell you about them, since if you're not responsible enough to find it out yourself you shouldn't be doing any drugs in the first place.
