In the middle ages, average life expectancy was around 30 years, maybe 40 if you were lucky. 50 year olds were considered exceptionally old.
And the thing about chronic illnesses, that's just it, most of them manifest only at older ages. When we now are able to have much longer lives, there's much more chance for us to develop these illnesses as well.
